If you do not have a NHS Wales App account, you can use this form to request any repeat prescriptions from the Practice.

Please allow 2 working days before collecting your prescription and please state in the comments box whether you would like to collect your prescription from Pengorof surgery/Ystalyfera surgery/ Abercrave surgery OR one of the local chemists.
