We ask you take your medical card to any of our three surgeries or you can complete the Registration Form GMS1w and either the Adult Health Questionnaire or the Child Health Questionnaire form in order to register with us.

Following registration with the practice all patients over the age of five years can request, or have requested on their behalf, a registration clinic for a routine health check.

We believe strongly in preventative medicine and our clinical staff are pleased to give advice on blood pressure checks, cervical smears and immunisations.

To register you will need to live within our practice boundary.
